KISASA NAILS — Booking Consent, Health & Safety Acknowledgment

Effective September 5, 2026 · Applies to all KISASA NAILS appointments

KISASA NAILS provides cosmetic nail and pedicure services only. We may decline, modify, or stop a service when doing so is necessary for health, safety, sanitation, or regulatory compliance.

Client information required at booking

  • Mobile number and email address
  • Selected service
  • Date of birth or age confirmation
  • Emergency contact (recommended for longer services)
  • Allergies, sensitivities, or relevant conditions that may affect a cosmetic nail or pedicure service

Health & safety screening

Do you currently have, or have you recently had, any open cut, bleeding, rash, irritation, inflammation, suspected infection, fungus, wart, unusual nail lifting/separation, contagious illness symptoms, or a known allergy/sensitivity that could affect this service?

If you answer “Yes,” KISASA NAILS may require a service change, reschedule, or decline the appointment. KISASA NAILS does not diagnose or treat medical conditions. Please seek care from a qualified healthcare professional when appropriate.

Required acknowledgments

1. Age and authority

I am at least 18 years old and completing this form for myself; or I am the parent/legal guardian completing this form for a minor. I understand a minor’s parent/legal guardian must accompany and remain with the minor for the full appointment.

2. Health information

I have truthfully disclosed any condition, allergy, sensitivity, product reaction, medication, injury, or concern that could affect the service. I will immediately tell the technician if I experience pain, burning, itching, swelling, dizziness, discomfort, or any other adverse reaction during the appointment.

3. Cosmetic-service risks

I understand that cosmetic nail and pedicure services may involve products such as acrylic, gel, hard/builder gel, polish, adhesives, acetone, lotions, soaps, disinfectants, and related tools. Even with appropriate sanitation and care, possible risks include temporary redness, irritation, sensitivity, allergic reaction, product lifting, nail breakage, or damage to the natural nail. Individual results and wear time vary.

4. Safety-based refusal or modification

I understand KISASA NAILS will not perform services over open wounds, irritation, inflammation, suspected infection, or a condition that appears unsafe to service. I consent to the technician modifying, pausing, or ending the service when reasonably needed for safety or sanitation.

5. Scope of service and aftercare

I understand these are cosmetic services only—not medical treatment. I agree to follow the aftercare instructions provided to me. I will contact KISASA NAILS promptly about a service concern and will not attempt an unsafe repair or removal before receiving guidance.

6. Booking and private-studio policies

I understand that my appointment is by appointment only and is for the booked client only; no additional guests are permitted. I have read and agree to the current deposit, cancellation/rescheduling, late-arrival, no-show, payment, repair/refund, and studio-access policies shown in the booking flow before I submit my appointment.

7. Assumption of ordinary service risks

I voluntarily choose the requested cosmetic service and accept the ordinary, disclosed risks described above. To the fullest extent permitted by applicable law, I accept responsibility for consequences caused by information I did not disclose or by failure to follow aftercare. Nothing in this acknowledgment asks me to waive rights related to conduct that cannot legally be waived, including gross negligence, recklessness, intentional misconduct, or non-waivable legal duties.

Optional portfolio/photo consent (optional)

I authorize KISASA NAILS to take and use close-up photos or video of my nails, hands, or feet for its portfolio, social media, website, and promotional materials. My face, name, contact information, and identifying details will not be used unless I separately authorize them. I may revoke this permission for future use by contacting KISASA NAILS in writing.

Professional-status disclosure

Services are performed by a nail technician student with 3 years of experience. KISASA NAILS is not a licensed salon; services are provided for cosmetic and practice purposes, and clients acknowledge this disclosure before booking.

Electronic agreement

By checking the required boxes, entering my full name, and submitting my booking, I confirm that I read and agree to this form and intend this to be my electronic signature.

Booking Policies

Deposit Required

Your appointment is not confirmed or reserved until your required deposit has been received and verified.

Please submit your deposit using one of the approved payment methods provided during booking. Include your full name and appointment date in the payment note.

Until your deposit is confirmed, the requested appointment time will remain available and may be reserved by another client who completes the booking process and pays the required deposit first. Appointment times are secured on a first-paid, first-confirmed basis.

Private Studio Guest Policy

KISASA NAILS provides a private, one-on-one studio experience. For everyone's comfort, privacy, and safety, only the person receiving the scheduled service may enter the studio.

Exceptions are limited to the following:

  • Two clients who have scheduled back-to-back appointments may arrive together. Both appointments must be confirmed with the required deposits.
  • A client who is a minor must be accompanied by a parent or responsible adult who remains present throughout the service.

Please do not bring children, friends, family members, or other additional guests who are not receiving a scheduled service.

Studio Location

Will be sent in a separate email and/or text message directly to the client.

KISASA NAILS is a professionally designed private nail studio located behind a gated residence within a West Mobile subdivision. We are not a traditional storefront or walk-in salon.

All services are provided by appointment only. For privacy and security, complete arrival and entry instructions will be provided after your appointment and deposit have been confirmed.

By submitting your deposit, you acknowledge and agree to follow the appointment, guest, cancellation, and studio-entry policies. Once your deposit is verified, you will receive your official appointment confirmation and arrival instructions.
